NHS Dorset Clinical Commissioning Group (CCG) is calling for anyone who has had experience of end of life care at home over the last three years to come forward and give their opinions.
This feedback will help shape the future of palliative care in the community services across Dorset.
With the number of people dying in hospital reducing and increasing in other settings as well as at home, it is important that patients and carers across Dorset have consistent access to quality care with a focus on what really matters to them.
The CCG has formed a Palliative Care in the Community working group to explore and develop a potential palliative care in the community service.
Partners in this group include: BCP County Council, Domiciliary Care providers, Dorset Healthcare, Forest Holme Hospice, Lewis-Manning Hospice Care, Macmillan Unit, Marie Curie, University Hospitals Dorset NHS Foundation Trust, Weldmar Hospicecare.
Your opinion matters, the importance of understanding what matters to patients and families who have experience of end of life care, as part of the planning process for this new service is key.
